As part of the voluntary community midwifery care program with a supervised learning nature, I created a personal data protection (GDPR) information and consent form.

This form aims to:

  • to ensure transparency and trust between midwife, student and family,
  • clearly define the role and responsibility of each participant,
  • and to protect the personal data and privacy of the woman or family receiving care.

It is an indicative and practical tool, designed to facilitate midwives who wish to pave the way for students and new colleagues, offering them the opportunity to learn about the philosophy of community midwifery within a safe and structured framework.

This document is provided solely for informational purposes and as a basis for inspiration. It does not constitute legal advice and has not been reviewed by a lawyer. Each midwife must adapt it to her own work context, procedures, collaborations and the requirements of the legislation that governs her. The author is not responsible for any inaccuracies, omissions or consequences that may arise from its use or modification.

The form is based on the current European and Greek legal framework (GDPR) and can be a useful starting point for midwives and students participating in respective observation, practical or community midwifery care programs.
